Nominations for Recognition Awards:

Guidelines
The Awards Committee of NCURA Region IV invites your participation in selecting and recognizing members and/or programs within our region deserving of special recognition in one of the following five (5) award categories: We are requesting that you identify those you believe to be the most deserving candidate(s) within each category. Nominations may be mailed or faxed, but must include the Nomination Cover Page and a 300 to 500 word statement that addresses each criterion for a specific category. For award categories #1 and #2, please provide the nominee’s curriculum vitae. Any supporting letters should address the respective award eligibility criteria. We welcome self-nomination in the categories of Outstanding New Professional and Meritorious Contributions and Special Merit and Distinction. Meritorious Contribution nominations may include team or office efforts, as well as those of a single individual. In the case of a self-nomination, a list of four references should be included in lieu of support letters. Each awardee will receive a commemorative plaque and be publicly recognized at the April 2009 Awards Luncheon to be held during our Region IV Spring Meeting in Minneapolis, MN.
